Well-Child Visits

Children’s Wellness Center places tremendous emphasis on the value of well child care. Well child checkups are a series of recommended checkups that are performed periodically throughout your child’s life beginning from birth through adolescence. We follow the recommended schedule as outlined by the American Academy of Pediatrics. If your child is scheduled for a well child checkup then we would like for you to come prepared and know what to expect at each of these visits.

Our office is proud to be following the Bright Futures Guidelines for our well child checkups as recommended by the American Academy of Pediatrics. As your child’s healthcare provider, we strive for positive health outcomes for all our families by promoting health and education, 

anticipatory guidance, disease prevention and early detection of disease. In order to best ensure your child’s health and well-being, we ask that you come prepared to each of your child’s well care visits. Please ensure that you have confirmed your child’s appointment, login to our patient portal to fill out all recommended surveys assigned to you for your child’s checkup (so that we can assess developmental, safety, behavioral and health concerns), click on the appropriate tabs below to know what to expect at the visit, bring any school or camp forms that you need to have filled out and lastly, write down any concerns or questions you want to make sure we address. After the visit, you may schedule your child’s next well visit and remember to log back in to our website so you can download the valuable resources that we have made available to you as well as review the Vaccine Information Sheets (VIS sheets) for the vaccinations that your child has received.

We have outlined below the recommended checkup schedule and what to expect at each of the checkups. Please click on the tab to find information regarding the visit, screenings and procedures that are will be performed at the visit, and vaccinations that are performed. You will also find valuable resources, educational materials, anticipatory guidance as recommended in the Bright Futures guidelines as well as the Vaccine Information Sheets for the vaccines that will be performed at their visit. We strongly recommend that you download and review these resources. These resources are age specific and will target on age appropriate anticipatory guidance, safety, development and the promotion of health and mental wellness.

Schedule of Visits

NEWBORN CHECK UP

Congratulations! Your baby is finally here. Newborns are wonderful, but exhausting! Having friends or family around to provide extra help and support can be very helpful. Make sure all visitors wash their hands well. Your baby’s first visit to our office takes place during his or her first week of life, usually 1-2 days after you are discharged from the hospital.

We do a lot at this visit, including:

 

  • Check your baby’s weight.

  • Monitor for signs of jaundice.

  • Check to see how your baby is feeding.

  • Answer questions about your new baby.

  • Discuss SIDS prevention, tummy time, newborn care

  • Review Newborn paperwork including newborn hearing screen

  • Review important Family medical and Social history

  • Set up access for Phreesia

  • Reviewed emergency symptoms and where to seek care

Immunizations: HepB (if not given in hospital)
Tests/Screens/Assessments: bilirubin level if jaundiced

1 MONTH WELL CHILD CHECK

Families are usually settling in some by this visit. Feeding, sleeping and bowel habits are sometimes a concern. At this visit we will track your baby’s growth and development and answer any questions.

We will review:

​

  • Newborn metabolic screening results

  • Newborn hearing screen results

  • Post partum depression screening

  • Discuss normal fussy periods at this age, safe sleeping habits, night time routines

Tests/Screens/Assessments: Post Partum Depression Assessment (EPDS)

2 MONTH WELL CHILD CHECK

Many babies seem more fussy or colicky at this age. Be patient. Colicky babies eventually outgrow their crying phase. Keeping your baby safe is the most important thing you can do. Even if you feel frustrated, handle your baby gently and reach out for help to friends, relatives or your pediatrician if the crying is getting to be too much. If you need a break and no one is available to help you, place your baby on his/her back gently in a safe space such as a crib or bassinet and take a few minutes to calm down.

We do a lot at this visit, including:

​

  • Measure your baby’s weight, length and head size and record on a growth curve

  • Provide your baby’s first set of immunizations. Immunizations are safe, effective, and keep your child and our community healthy. During this visit, you will be able to discuss any questions you may have about immunizations

  • Post partum depression screening

  • Discuss feedings, schedules, night time routines, safe sleeping (now is the time to discontinue swaddling)

4 MONTH WELL CHILD CHECK

Your baby is becoming more active and verbal. Be careful not to leave your baby unattended, especially on a bed, couch, or counter. They move faster than you think! Babies need only breast milk or infant formula for the first 4-6 months of life. We will talk at this appointment about the appropriate time to introduce solid foods and what foods to introduce first. We will also discuss prevention of sleep problems and allowing your infant to self sooth.

At this visit we will:

  • Measure your baby’s weight, length and head size and record on a growth curve

  • Provide your baby’s immunizations

  • Discuss when to start feeding your baby solid foods

  • Screen for post-partum depression

  • Review developmental screening and any concerns

6 MONTH WELL CHILD CHECK

Your baby will begin to move more and will be able to sit up independently for short periods of time. Now is the time to start baby-proofing your house if you haven’t already done so. You should move the mattress in your baby’s crib down to the lowest level.

​

At this visit we will:

 

  • Measure your baby’s weight, length and head size and record on a growth curve.

  • Provide your baby’s immunizations

  • Discuss Oral Care and ensure adequate fluoride intake ( if you use city water then it already contains adequate fluoride)

  • Discuss introduction of solid foods and including introduction of highly allergenic foods such as peanut products

  • Discuss safety proofing and safety concerns

  • Screen for post partum depression and address and concerns

  • Review developmental screening and address any concerns

Immunizations: DTaP, HIB, PCV13 and Rotavirus. Flu Shot (during flu season)

9 MONTH WELL CHILD CHECK

Your baby may be starting to crawl, pulling to a stand, and cruising on furniture. Babies will want to touch everything; this is how they learn. This is a messy time! Have fun and be patient.

​

At this visit we will:

​

  • Measure your baby’s weight, length and head size and record on a growth curve.

  • Discuss your baby’s dental care. You should be brushing your child’s teeth with an infant toothbrush or a finger brush and should not be feeding overnight.

  • Discuss self feeding and finger foods

  • Review developmental screen and address any concerns

  • Review Safety survey as well as Health risks including lead exposure and tuberculosis risk assessments

Tests/Screens/Assessments: ASQ developmental questionnaire,Safety, lead screening test (if high risk)

1 YEAR WELL CHILD CHECK

Your child may be able to climb and run, so be sure to safety proof your home. Poison control is available 24 hours a day: 1-800-222-1222 – keep this number handy in your cell phone and on your refrigerator.

At this visit we will:

​

  • Measure your baby’s weight, length and head size and record on a growth curve.

  • Provide your baby’s immunizations

  • Review developmental survey and concerns

  • Review developmental screen and address any concerns

  • Discuss introduction of whole cow’s milk and calcium/vitamin D intake

​

Immunizations: PCV13, Hepatitis A, Varivax and MMR vaccines
Tests/Screens/Assessments: ASQ developmental screen, Hemoglobin test, Lead screening test (these will involve a toe prick), Vision screen (instrument based photo screener)

15 MONTH WELL CHILD CHECK

You most likely have a busy toddler now! Keeping your eyes and ears on him is a must… make sure your toddler’s surroundings are safety proofed so they can continue to explore! At this visit we we will be discussing your toddler’s nutrition, growth and development. At this age it is important to foster their independence but set limits and don’t worry temper tantrums are normal and expected.

At this visit we will:

Immunizations: HIB vaccine
Tests/Screens/Assessments: ASQ developmental survey, EPSDT via Phreesia

18 MONTH WELL CHILD CHECK

Your toddler is probably pretty demanding at this time and full of energy and curiosity! This is also the time they begin to do imaginary play and may have even learned some of their own body parts. We will review their growth and any developmental concerns. It is important that we keep safety a priority, encourage play by singing and dancing with them and also make sure we are doing appropriate dental care.

At this visit we will:

Immunizations: DTAP, IPV, HEP A vaccines
Tests/Screens/Assessments: ASQ developmental survey, MCHAT-R and EPSDT via Phreesia

2 YEARS WELL CHILD CHECK

Your toddler is by now talking and communicating more! They are probably starting to string a few words together. Toddlers are full of imagination, energy and attitude! We will review their growth and development. Meal times can be challenging as toddlers tend to be picky eaters. We will also discuss potty training, limiting screen time and toddler behavior.

At this visit we will:
 

  • Hg (test for anemia) and Plusoptix (photo screener)​

  • ​Lead screening and PPD will only be done on those that are high risk
     

Immunizations: None
Tests/Screens/Assessments: ASQ developmental survey, MCHAT-R screening, EPSDT, Safety survey via Phreesia


RESOURCES TO DOWNLOAD:

2.5 YEARS WELL CHILD CHECK

Your baby is not really a baby anymore. They are firmly in the toddler years. One moment they seem so big and the next still very small. This is the “do-it-myself time.” Your toddler has boundless energy and personality at this stage! This is an important time to let them be creative and use their imagination. Reading, playing games, nursery rhymes and dancing together are all fun activities to share together.

At this visit we will:

  • on the portal complete the ASQ Developmental survey and the MCHAT survey​

  • ​procedures in office: none unless they have been found to be high risk on screening questions then we may perform vision and/ or Hg check to assess for anemia

​

Immunizations: None
Tests/Screens/Assessments: ASQ developmental survey, MCHAT-R screening


RESOURCES TO DOWNLOAD:

3 YEARS WELL CHILD CHECK

Your child will likely be moving into a world of make believe that they create with their imagination. Toys for dress up and make believe are important at this age. Read books, sing songs and play rhyming games with your child each day.
Your toddler may have already started preschool or is busy watching and interested in what other children are playing. They certainly like to be independent as well! At this stage, it is also important to set limits and realize the importance of being able to say “No” to your toddler. If your child, has not had their first dental appointment this is a good time to set up you dental home.


At this visit we will:

​

  • on the portal complete ASQ developmental survey and the EPSDT health risk survey

  • procedures in office: Hg ( to assess for anemia) and plusoptix (visual photoscreener). Lead screening and/or PPD test will be placed if found to be high risk

Tests/Screens/Assessments: ASQ developmental survey, BESCA, MCHAT-R screening, EPSDT, Safety survey via Phreesia
