Please Note

Patient Notice: Physical forms, 3231, 3300, camp forms, and school sport forms have a turn-around time of 7-14 days. Faster turn-around requests will incur a $15 rush fee.

We require 24 hours notice for cancellations of all appointments. Failure to do so will result in a $50 fee, due before your next visit.
